ENROLLMENT CHECKLIST
Before your child can begin care, I must have all of the following:
1. Completed paperwork:
· Copy of current immunization records signed by a health care professional. Child must be up to date on immunizations.
· Health statement signed by a healthcare professional.
· ALLERGY ACTION PLAN completed by health care professional IF your
child has FOOD or other SEVERE ALLERGIES.
· Completed Enrollment form
· Completed Child Assessment form
· Completed Food Program Enrollment form
2. Enrollment fee ($100.00)
3. First week's fee. Payment for the first week is due no later than the Friday before your child's first day (and subsequent payments are due each Friday for the following week). Currently, rates are 200.00-220.00 per week.
Before your child’s first day, you need to bring:
1. Completed paperwork
2. Change of clothes including socks
3. Diapers (or underwear)
